Elderly Waiver Services and Provider Info

Care Coordinators play an important role in helping Blue Plus members stay healthy and independent. As an extension of the care team, you are in a unique position to offer important education, motivation, and support to our most vulnerable members around the topic of fall prevention. We are providing you with important information about falls and fall risks. In addition, we have developed a Fall Prevention Conversation Guide to help you initiate important conversations with older adults (65+ years) about how to reduce the risk of falling. Home Assessment and Modification:
The Care Coordinator should always confirm Blue Plus network status with the provider. When it is necessary to use a provider that is registered w/DHS but is not in the Blue Plus network, the Care Coordinator must add to the service agreement description “Out of Network”. These provider lists are a moment in time and are subject to change.

Medicare Part D Claim Reimbursement Form:
- Form for members/Care Coordinators to request reimbursement for:
- Part D covered vaccines received in a clinic setting or
- Self-administered medications while in a hospital during an observation stay or in the Emergency Room
- Clinics/hospitals cannot bill certain Part D covered drugs. Members will get billed and can 1) pay the bill and submit for reimbursement or 2) not pay the bill and submit invoice for reimbursement. Ask your PR Consultant if you have any questions.
- Form can be mailed to address on the form or faxed to 1-800-693-6703.
- Must include copy of paid receipt or unpaid invoice.
- MTM Services are provided telephonically by in-house pharmacists who can complete a Comprehensive Medication Review for our MSHO Members.
