Bridgeview Tuesdays are cancelled until further notice.
Known defects post 2/26/2025 webtool updates:
- Care Coordination (T1016 UC) (G9002) and Case Aide service (T1016 TF UC) agreements are not correctly accumulating towards the case mix budget. Best practice and work around: Use separate tool to monitor service costs to ensure authorized services stay within the case mix cap.
- Date of Death (DOD) entry in the webtool is not automatically ending active services agreements. Work Around: Manually update active service agreement(s) and LTCC Case Mix span when making an entry to capture the date of death.
- MSC+ members are incorrectly displaying “Grace” on the member selection page. Work around: Disregard this incorrect grace period alert, MSC+ members do not have a grace period.
*Anticipated defect fixes will be on or after 3/12/2025.
Important: Do NOT modify existing transition PCA service agreements to add the additional 3 months of PCA service to agreements. Separate service agreements must be added for the additional 3 months for PCA and PCA supervision.

After the service-agreement is entered, complete and send the following forms as applicable to the region.

- Member address
