Reminders:
- Do not modify existing PCA or PCA supervision service agreements for members that are eligible for a service extension under the Transition Period for CFSS extension. New service agreement(s) must be created in 6 month increment(s), one at a time, not to exceed 9/30/2026. Refer to the CFSS communiques for more details.
- Do not create service agreements in Bridgeview for CFSS providers that are not listed on the CFSS Provider List located on the Care Coordination tab under CFSS.

For Care Coordination Delegates not able to use the county pass-through option, see the Bridgeview Care Coordination Delegate User-Guide for how to enter non-medical EW bus passes in each region. After the service-agreement is entered, complete and send the following forms as applicable to the region.

NE Region:
- Arrowhead Transit Bus Pass Request Form – All County BCBS
- Arrowhead Transit Bus Pass Request Form – Ely, Aitkin, Moose Lake, Floodwood, Grand Marais, Grand Portage BCBS
Metro:
The direct link to Metro Transit Go-To Card is: https://www.metrotransit.org/go-to-card. Metro Transit Customer Care Number: 612-373-3333. Must have 16 digit serial (pass) number on the back of the card.

Required information when requesting a replacement card:
- Member First and Last Name
- 8+PMI
- Member address
- Serial (pass) number on the back of the card (16 digit number), if known
