Information and resources from ACT on Alzheimer’s as presented during the Blue Plus Webinar: Best Practice: Simplify and Optimize Dementia Care held on 6/25/15. ACT on Alzheimer’s is a volunteer-driven collaboration bringing Minnesotans together to create supportive environments for everyone touched by Alzheimer’s disease and prepare Minnesota for its impacts. Their site provides a toolkit and resources to guide communities in becoming dementia friendly, practice tools to assist professionals in identifying and managing the disease and an economic model of dementia intervention with projected cost savings.

Adult Protection
Information about the MN Adult Protection Program operated by DHS. The APS unit provides consultation and training to County AP staff regarding their responsibilities under Minnesota Statute (626.557, 626.5571, 626.5572) and Minnesota Rule (9555.7100-7700). They also provide additional consultation to families, providers, and other stakeholders around Vulnerable Adult (VA) issues.

The Exchange is a partnership formed in Minnesota to exchange information and resources about health communication and to share multilingual health materials. A Risk Management Plan is a document that prepares to foresee risks, estimate impacts, and define responses to issues. A risk is “an uncertain event or condition that, if it occurs, has a positive or negative effect. The risk management plan contains an analysis of likely risks with both high and low impact, as well as mitigation strategies.
