Data Privacy Incident Information

Some provider information is considered private data/nonpublic data under the Minnesota Government Data Practices Act (MGDPA). While National Provider Numbers (NPIs) are public data, Unique Minnesota Provider Identifiers (UMPIs) are private.

Currently, UMPIs are displaying on support plan printouts when they should not. DHS is working to implement a fix in the MnCHOICES application.

Call to action: Until the fix is implemented, all users must redact Provider Identification Number (NPI/UMPI) beginning with an “A” or “M” before providing the support plan print out.

Important: 

All lead agencies must complete all assessments and support planning activities in the MnCHOICES “2.0” application, with the exception of a member residing in the nursing home and intermediate care facility and use of the Blue Plus NH-ICF Member Assessment and Support Plan. Reminder: All return to community assessments must be completed in MnCHOICES “2.0”. Legacy systems have been retired. 

All Care Coordinators, except physician assistants, nurse practitioners and physicians acting as Care Coordinators for members in nursing homes, must be certified assessors to complete assessments in the MnCHOICES “2.0” application. Certified assessors must provide the assessment and ongoing care coordination functions for the member including support planning. The Certified Assessor and Care Coordinator must be the same person.

MnCHOICES MNCAT training for continuous learning units (CLU) is required and must be monitored by the contracted delegate agency and made available upon request.

MnCHOICES Office Hours: Occurs the first Friday of every other month 9:30 to 11a.m.

Purpose is to provide technical assistance, communications, and support to lead agencies as they implement and manage MnCHOICES applications. Questions can be submitted in advance. Open to anyone, but mentors and administrative contacts encouraged to attend.
