MnCHOICES Mentor(s)

Definition: The MnCHOICES Mentor (s) selected for your delegate agency will assist staff who use MnCHOICES. The MnCHOICES Mentor (s) is considered the subject matter expert user of the MnCHOICES application and will provide troubleshooting and other user support to their agency staff. The number of Mentor (s) may vary per delegate agency based on the size, staffing and resources.

The MnCHOICES Mentor(s) are expected to attend the trainings provided by DHS (no additional training will be provided by Blue Plus). Mentor(s) are the agencies principal contact and resource person for training and disseminating communication and information to the user(s) within your delegate agency, in addition to:

  • Support your agency by sharing relevant information (including outage messages, release notes and other information) before, during and after launch
  • Assist in coaching agency staff in developing MnCHOICES skills and proficiency
  • Provide hands on support for the agency’s MnCHOICES users
  • Identify and assist with issues/concerns through problem solving and consultation with your supervisor and/or Partner Relations Consultant
  • Attend MnCHOICES meetings and trainings
  • Sign up for the MnCHOICES Matters Newsletters using the online form (open to all MnCHOICES users)

Update on launch of MnCHOICES Revision Project 

Launch schedule details

PHASE 4 (FINAL TRANSITION PERIOD) — JULY 1, 2024   TO SEPTEMBER 30, 2024

Lead agencies should:

  • Have 100% of staff members conduct all work in the MnCHOICES revision.
  • Finish all existing assessments and support plans in legacy systems by Sept. 30, 2024. MnCHOICES 1.0 assessments and support plans not completed by Sept. 30, 2024, will not migrate to the MnCHOICES revision. As a Mentor/Supervisor, please review MnSP support plans for your delegate agency for any “open” documents. Your agency needs to finalize and move to close/complete status prior to 9/30/2024. Priority should be given to the oldest plans first and working your way to current plans in MnSP.

Effective Oct. 1, 2024, legacy systems, including MnCHOICES 1.0, will no longer be available.

ExceptionSix counties currently contract with public health nurse agencies that use the legacy PCA/CFSS document to conduct PCA/CFSS reassessments. DHS will work with these counties to move their work to the MnCHOICES revision after Sept. 30, 2024
